Social Services Case Manager
Reporting to the Family Wellness Manager, the Case Manager works within the clinical department teams during the Behavioral Health Integration project to provide and coordinate social services support for patients through the development of an action plan with the patient to achieve the patient’s desired goals. He/She will provide culturally and developmentally appropriate needs assessments, health education, information and outreach to DHMSC’s clients/patients. Works in collaboration with behavioral health clinicians, nurse case managers, providers, and other staff to coordinate services that will improve the overall health outcome of the patients/clients, such as:
- Writing advocacy letters;
- Enrolling in income support programs, & obtaining food security
- Providing service referrals (childcare, housing, external BH care, etc),
- Coordinating transportation to/from appointments education,
- Workforce training and job placement,
- Immigration,
- Facilitating group visits.
Other responsibilities may include arranging prior authorizations for patients as required when accepting behavioral health care and working collaboratively with other Behavioral Health Integration (BHI) project staff to support project goals to increase access to mental health counseling through primary care visits. Demonstrates experience in procuring approval for behavioral health visits
Experience and/or Education Desired:
BA/BS preferred; HS Diploma required;
Minimum 2-5 years related experience required;
Experience working with families from diverse backgrounds;
Bilingual/bi-cultural highly preferred;
Excellent organizational, communication and writing skills are a must;
Must have knowledge of community organizations, government agencies and the public benefits system.
Special Qualifications and Remarks: Must have a Mass driver’s license and a working car, a Criminal Offenders Records Information (CORI request must be completed for this position.
